Instructions for Lafayette policy holders to request loans
1. Log into your Lafayette Portal
2. Select “Request a Loan”
3. Select the policy that you’d like to take a loan from
*You may need to have your spouses signature. If you have multiple owners to your policy, you’ll need to print off the form.
4. Write in an amount (the system will show you the maximum amount that you can take)
5. If receiving funds digitally, confirm last 4 of account number.
6. Check takes typically 7-10 days, Digital is typically 2-3 days.
7. Print your name, put in the date and confirm.
*any amount over $50,000 will require the form found on “Making Policy Changes“.
*If you’ve submitted form 1724 you will be able to select a direct deposit. If you haven’t set that up, click here to download Form 1724 and follow the following instructions:
Setting up electronic direct deposit
- Download and print Form 1724
- Fill out as per the instructions on the form.
- Attach a picture of a voided check for the account where the loan will be deposited
- Make sure it is an ink (“wet”) signature.
- Email form to [email protected] and CC [email protected]
- Call Policy Services (1-800-243-6631) the next day to ensure they received it and
everything is in order. Get an ETA on when the bank account info will be uploaded to
your portal so you can request a loan.*If your spouse is a joint account holder for the bank account, you will need his/her
signature on the form as well.
A note on interest
- You cannot pay your interest online. You need to send a check into the company and in
the memo line put “[policy #] Loan Interest” - You should receive an interest payment invoice in the mail separately from your annual
premium invoice. You need to pay them separately. - If you don’t pay your interest off prior to your anniversary date, the interest will be added
to the principal balance of your loan. Any loan repayments you make will go directly
towards the principal balance of your loan.
